Once your appointment is confirmed, your intake forms arrive by email. Completing them at home means your visit begins with care rather than a clipboard.

Mary Yegoryan, M.D., M.S., PA-C reviews your history, examines your skin, and listens to what you want changed. Questions are welcome, and nothing is rushed.

You go home with clear next steps: what we treat now, what waits, what to use daily, and when to return.
Good to know
Rashes, acne, moles, and skin checks are medical visits and are billed through insurance where your plan is in network. We are currently contracting with most major plans, so please confirm your coverage with our office. Botox, filler, peels, and PRP are cosmetic and are self pay. If you are unsure, book the medical visit and we will guide you from there.
Often yes. Many cosmetic consultations can be treated during the same appointment when it is medically appropriate and time allows. Mention it when you book so we can hold the extra time.
We accept major credit cards and cash. We are currently contracting with most major insurance plans and expect to participate with a broad range of insurance companies. Insurance participation is subject to active contracts and plan-specific benefits. Please contact our office to confirm your coverage.
We ask for 48 hours notice so another patient can use the time. Life happens, so call us as soon as you know and we will find you a new spot.
Come without makeup if you can, skip alcohol and blood thinning supplements for two days when your doctor allows, and pause strong retinoids or acids for three days before peels or microneedling.
If you have had a biopsy, surgery, or treatment elsewhere, bring the report or your prior product list. It saves repeating steps that did not work.
